I had a co-teacher who had this book and we used it constantly. The part that we found to be very effective was the "Student Behavior Chart". We would write behaviors (up to 3) that we wanted to see stop. The parent were involved by signing the paper nightly and all teachers would initial after the class was done. It was the student responsibility to have it signed. We would have the students return it daily and 4 out 5 days they would receive an incentive. If it was not signed, it meant a lunch detention first then a call home. The kids would ask for them when they thought they needed some behavior changes. The maximum any student was on the chart was 6 months.
